    Service provided on April 7 2023, *************************** got referred to Quest diagnostics for some lab work, referring Physician was ********************* PA from Palmer Family Medicine. The claim was denied by ****************** due to the claim comments on bill from Quest that the referring Physician ********************* NPI Number is registered in Ga not ******. I have been trying to get this issue resolved since April. Palmer Family Medicine refuses to provide an ****** *** # for ********************* from the get-go. Telling me that she has one and that the problem is with my insurance and Quest. I have spoken to both Quest and Premera, fact is the problem goes back to Palmer Family Medicine does not have an ****** *** # for ********************* for the *************** and I have been told numerous times that she does have one. I went online to the public site that one can pull up all *** Numbers for the *************** and she is not register in ******. This is the second time it has happened through their medical office. Now I am getting a notice for collections for this invoice. Quest has an ****** territory agent that has been trying to help me resolve the issue by also calling the Palmer Family Medicine office asking for the *** # and they lied to him also, saying they've applied and don't know what is holding it up. Today I called today 12/27/23 and was spoken to by an office personnel in a very rude and unprofessional way. Basically, telling me that it was my problem not theirs. we ae a retired couple on a fixed income and every ***** counts. We don't mind paying our bills and always have, but paying something we don't owe due to someone else's mistake is unacceptable. Please can you assist us with this problem? Much appreciated.     This seems to be a major misunderstanding when it comes to Quest billing. All of the lab work ordered via providers at Palmer Family Medicine are completed at an outside lab. We do not handle ANY billing for outside labs. The *** number that was requested is a national number attached to each individual provider. Said *** number is on every Quest requisition and is on file with Quest Diagnostics. ****************** *** number was provided to the quest diagnostics manager many times as it was also offered to the consumer multiple times via phone call, by myself personally.  After not having this brought to our attention until early October. I have been researching this and working along with the Quest billing service manager and spending hours on the phone with *******. We were informed by the insurance company that they had written a check to the covered party in August of 2023 in the amount of $383.21 to cover the insurance companys portion of the lab work that was completed. Leaving the patient, a balance of $88.07.Per the insurance company there was never an issue regarding the *** number on file. The consumer verified on a phone call that they did receive a check and that said check was deposited into their personal account. The insurance company reported that the check did clear via the bank on October 11 2023. We did attempt to have a productive and professional conversation with the consumer sadly she was not responsive to hearing said conversation and was acting in a argumentative manner. I feel as the consumer received payment by Premera that this is a resolved issue that the consumer should be willing to pay for services rendered by Quest diagnostics. We do pride ourselves in our willingness to care for and go above and beyond for our patients at Palmer Family Medicine and I feel that this is not an issue with our company, simply a misunderstanding by the consumer when it comes to a personal insurance plan and an outside laboratory.
