      I gave the service provider my insurance card and provided them proof of who I was. I went into this urgent care center to have an evaluation on a lump that needed to be removed on my breast. I have had the same insurance since 2007. I just noticed on a credit report this company put me into collections for a balance of $****** which would have been paid in full by the insurance company. I proceeded to call to ask them why I was not notified there was an open balance; how else would I know if there was anything due. I was told the insurance company didn't verify the insured being me. I verified with my insurance provider and they said this is not true there are recorded calls and no one ever presented the claim. I am thankful I review my credit and my bills and take pride in remaining to have a clean record; I feel this is unjust of this merchant/vendor/doctors office to impede on my good credit and not notify me to rectify this

      I became ill while visiting my house in ****** Alabama. I contacted a family member who is an EMT and they diagnosed me with hyponatremia which means your body has taken on more water than your kidneys can process. They suggested I request a saline infusion. So I went to American family care in ****** and explain to them I was self diagnosing and asked if they could give me ******. They left the room to consult came back and said no they could not but they could offer me a Covid test and several other tests to see what my problem was. I declined the tests went to the front desk and they said OK fine have a nice day. A month later I received a bill stating I had received all of the tests they suggested. I did not receive any test or any services of any kind.
