      Hello, I'd like ****, the Physical Therapist I worked with at Foothills Sports Medicine's Biltmore location, to correct the discrepant information he entered on the evaluation in November 2024, which is negatively impacting my ongoing Personal Injury Case. He can either work with my attorney or contact me directly if necessary to resolve the ********* the appointment, 1) I explicitly expressed that I did not and could not perform any physical activities besides light cardio, which was what I exercised at the gym; He asked me what types of motion aggravated my pain the most, I described the deadlift motions to demonstrate; 2) I mentioned that the pain remained the same intensity despite following his instructions taught at the initial appointment and extensive home care regime; 3) lastly, I had two different referrals for both my lower left lumbar and right shoulder, which I explicitly mentioned to both **** and the front desk staff only the lower left lumbar pertains to the Personal Injury case and I would like them documented separately.Instead, **** in error combined and misrepresented statement 1) and statement 2), and completely disregarded statement 3) in this quote obtained by my attorney directly from the practice: "Assessment: ***** is a very pleasant 25 year-old male banker. He presents with signs of symptoms of left lumbar strain stemming back from motor vehicle accident in early August. He recently re-aggravated at the gym doing dead lift activities. Also believes he aggravated his right shoulder. He currently has signs and symptoms of Rotator Cuff tendinitis, and mild impingement syndrome. Mr. ******* will benefit from a progression of skilled physiotherapy in order to achieve goals."The uploaded screenshots were my attempts to reach out to the practice via voicemail and email; I received no answer. I look forward to resolving this matter and thank you for your assistance.

      Over a period of many months, I was treated by Foothills Sports Medicine and Physical Therapy. During this time I was overcharged for services. I brought the issue to the front desk staff's attention. I was directed to contact the billing **** at ************. I spoke to someone the end of Nov about the issue. Then again 3 times in Dec...again in Jan twice... and in Feb twice with no resolution. ******* just hung up on me. They confirm they owe me the $172 and change, but keep giving me an excuse for why they have not issued the check in the last 3 months. I am tired of begging them for my hard earned money. I told *******. I am very frustrated and I really don't care about WHY their business practices aren't working to issue my refund, and she was obviously annoyed with me in her raised tone. I then told her the business practices of her business were non-reputable and I would be reporting to the BBB. She hung up on me. I just would like my money back with the 5.25% interest that it could have been earning in my savings account versus theirs. I think with my time and effort in repeated phone calls over several months, this is a reasonable request. Thank you for your help with this matter.
