    Subject: Complaint Regarding Lack of Documentation and Communication from Gratitude Lodge ***************************** Rehabilitaion Center To Whom It May Concern,I am submitting this complaint against Gratitude Lodge due to their failure to provide the necessary documentation and respond to my inquiries regarding the terms and costs of my treatment. Despite multiple attempts to resolve this directly with the facility, I have not received the information I am entitled to.I attended treatment at Gratitude Lodge from July 23, 2024 till about August 26, 2024. Since my discharge, I have made several requests for copies of my intake, discharge, and treatment records. These documents are critical for my personal records and any future healthcare needs. However, the facility has consistently failed to provide them.Additionally, I have tried to contact the facility to discuss the financial terms and costs associated with my treatment. Despite repeated phone calls and email inquiries, they have not responded or provided clear answers. This lack of transparency has caused significant frustration and uncertainty about my financial obligations.I believe this behavior is not only unprofessional but also potentially unethical, as patients have a right to access their medical records and understand the financial terms of their care.I kindly ask for the assistance of the Better Business Bureau in resolving this matter. I request that Gratitude Lodge provide:1. Copies of my intake, discharge, and treatment documentation.2. A detailed explanation of the terms of my treatment, including a breakdown of all associated costs.Thank you for your attention to this matter.
