    I sought care from Dr. *********. She insisted that I commit to a 'package' of treatments. I informed her that my case was very complicated due to a serious car accident, multiple surgeries plus chronic Lyme and Lupus. I was not comfortable with commiting to a 'package'. Appointments went from 7.23.21 through 10.4.21, when I stopped going to her office three times a week as I was only getting worse. Dr. ********* had spoken to me about a deep tissue therapist who she personally sought care from. I made an appointment with that therapist and realized enormous relief with just one visit. To this day, I continue to see that therpaist as needed. After two and 1/2 months of not seeing Dr. *********, I was contacted by her office and told I needed to come in, for an appointment and pay my bill. I said please let me know the amount I owe you and promised to stop by as soon as possible. (I live an hour away from her office.) When I arrived to pay the bill, Dr. ********* strongly suggested I get an adjustment, so I did. The front desk told me I owed $400 so I gave her a cash payment of $400 and she said everything was clear. A few weeks later, I began getting calls to pay the remainder of my bill. Dr. ********** said I commited to a series of treatments of which I received and I owed her. I explained that I was informed everything was paid. I then received a statement that, to me, made no sense as I was told everything was paid. Dr. ********** emailed me and called me and insisted that I owe her. She would not listen to the fact that I was told everything was paid. I did not hear from her until Sept 2022 when she charged my debit card. I called my bank and reported that it was not an authorized charge. Now she has filed a small claims suit against me with a court date of Nov 1, 2022. I feel her 'package pricing' is confusing if not a deceptive practice and only insures her an income without concern for the patient's benefit.

    RE: ******* ****** a *****, ID: ******** To Whom It May Co cern; I am submitting this response based on my records, notes, and correspondence regarding this matter. The patient came into my office for treatment on 23 July 2021. She sought care for pain in her right shoulder, neck pain, and radiating pain into her right arm and hand. She also complained of sleeping issues, weak muscles, and headaches and pain on the right side of her face. All the above complaints were due to an accident in the early 90's, as communicated by the patient. 
    New Patient Routine 
    for Chronic Pain 
    Patients either do int ke forms online or they have an option of doing them in the office. Forms that require signatur and approval are done in the office. 
    Exams are done on e first day of an appointment. Her exam was done on the day of her appointment, 23 Jul 2021. During an exam, orthopedic tests and posture evaluations are done. After the exa s are performed, the patient will be scheduled for a report of findings which takes place at later date. Ms. ***** followed this same routine. 
    Report of Findings (ROF) 
    During ROF's, I go o er all exam findings and posture evaluation findings. I also give my recommendations fo care. My treatments are based on three levels of care; crisis care, corrective care, and ellness care. Each level of care has a time frame attached. Depending on how the patient re ponds to the care, determines the next level. For example, crisis care is normally 6 weeks, c• rrective care normally 6 months, and wellness care can be one year. Again, depending on he patient outcome, determines the length of care. For Chronic patients, the care tends to be I• nger. Ms. ***** had a chronic condition expanding for years. If patients do not want the treat ent plan, they are given the option of paying the cash price for each visit. 
    I gave Ms. ***** my 
    commendation and she decided and agreed to move forward with the 
    three levels of care w ich consisted of a one year plan. Long term care plans can be costly, so

     I am rejecting this response because:

    Dr. ********* does not address the fact that I was informed a specific dollar amount per month covered specific treatments received.  
    I paid all agreed to monthly charges in cash, on time.  
    When, after 3 months of multiple appointments per week, my condition worsened, I discontinued treatment. 
    After she realized I was not returning for additional treatments, she began stating that I agreed to a yearlong program, which I did not.  I owed more money, which I do not.  There are multiple discrepancies with her statements, I have detialed these in a document attached.  

    Dr. ********* suggested I see a specific therapist; after only two sessions with this therapis, my pain was in control.  
    Dr. ********* was upset that I did not continue with her protocol.  
    She called me, interrogated me about attending his sessions and not hers.  
    She also contacted the therapist and complained.  
    I have never been treated by a chiropractor this way,  I am surprised and extremely surprised.  I too am a health professional and this, to me, it unacceptable behavior.

    Dr. ********* insists that she did not have my proper mailing address, then how did I receive, from her address, a debit card paid receipt in the mail, to the same address where the court document was served. Yet she insists that it cost her extra money to serve me because she did not have the correct address; she states that I neglected to fill out my entire address on her office forms.  Again makes no sense.

    To stop the parade of calls, texts and emails from this doctor and her office, I have offered to pay another $200 IF she agrees to stop all contact with me and to no longer gossip to, interrogate my therapist about my personal health choices and my life.

    Dear Ms. ********, 
    In regards to Ms. ***** rejection of my response, I am submitting to you my final response. 
    There are many false statements in Ms. ******* rebuttal that I will not respond to. This is just an attempt by her to further delay or negate her responsibilities. I made an reasonable offer to settle the debt. By getting you and your organization involved(only when  she has been served by the court), is her attempt to forgo her responsibility "she said, she said," and I refuse to waste anymore of your time responding to false  accusations. I stated all the facts in my previous letter. 

    Again, I made an honest attempt to rectify this through email and phone conversation. Ms. ***** stopped responding in April, and now, by her actions, did not plan on paying. I ask, did she have to involve your organization? My answer" of course not." A phone call, text or an email could have resolved this. I was willing to work with her as stated in the June email I sent. Again, no reply rom her, only a letter from you.

    In regards to her home address, we used the address that she gave us to serve her court documents. The person serving the documents told us it was not an address. My husband the did a google search and gave the server a business address. This is the address he used to serve her. This is really all I know about her address. I just know that I paid extra to serve her due to her address. 

    I will be presenting all documents in court where she will be allowed to dispute or accept what we present. Whatever the courts find, we will accept.
