I have never been to this place and they sent me to collections for $45. How do I get a bill somewhere Ive never been? I called billing and got absolutely nowhere. I call the front desk and they send me to billing. Looking at their ****** reviews, this is a chronic issue. Ive never seen so many complaints over the same thing. We sincerely apologize for the experience this patient has received from our company. We are a group of radiologists who read and interpret radiological exams for several hospitals in the ****** metro areas,as well as several mountain-towns. Our billing records show this patient had an exam done on 2/7/2023 that one of our radiologists interpreted at ****************************************. Our billing records also show the patient called on 12/13/2023 in regard to the statement balance. At that time, our billing company re-sent a copy of the billing statement, per her request. With the patient's permission, we can upload a copy of the ************************ Agreement that was signed prior to the medical services she received.

We sincerely apologize for the experience this patient has received from our company. After insurance adjudication, our billing data shows a coinsurance balance of $26.94. On 9/13/2023, a billing statement was mailed to the patient (same address listed in this complaint). On 10/13/2023, second billing statement was mailed to the patient. On 10/13/2023, the patient called our billing company stating he paid at the time of service. The billing representative informed him the billing statement was for the professional component (a fee separate from the facility/technical fees). Therefore, any fees he paid at the time of service did not include the professional component. On 10/27/2023, he received a phone call from our billing company to remind him of the outstanding bill, but there was no answer. On 10/30/2023, the patient called our billing company again. He said he processed a bill on 10/20/2023 through his credit union but was informed there is no record of any credit card payment on his account.
Our billing company performed additional research into this payment, but we still do not show receipt of his funds. We would gladly credit the account if the patient would provide us with a receipt with the payment details from his credit union.

Good afternoon,
We sincerely apologize for the experience this patient has received from our company. On 9/21/23, our billing company representative called this patient to get a better understanding of the difficulty she was experiencing with the phone number and online payment portal information printed on a statement she received. During their conversation, it was brought to the billing representatives attention that the invoice the patient was describing did not sound like (or have similar features) as the billing statements we send out. The billing representative asked the patient to email a copy of this statement to her directly so it could be reviewed, but the patient did not call back. On 9/25/23, our billing company representative called the patient again to follow up. At that time, the patient said she was out of town and would call her back the next day (9/26/23), but never did. Another voicemail was left on 9/27/23 but again, no call back was received. As of 10/11/23,our billing company still has not received a return phone call from this patient or a copy of the statement she is describing.

On September 6 I went for an ultra sound to Centura health. I was told that day they did not take my insurance but the cash price would be for $179 which I paid that day. Yesterday September 12 I got a bill from Colorado Imaging and Associates for additional funds. I have called Centura Health and Colorado Imaging and Associates being told that only the other company can help etc. There is nothing they can do. I was lied to. I only did not change my appointment because I was told it would cost $179 which was not true now I am being billed for more. This is fraud.Business Response

We sincerely apologize for the experience this patient received from our company. We are a private group of radiologists who are contracted to interpret/read radiological exams for Advent Health and Common Spirit hospitals in our area. After reviewing the patients account for services rendered on 9/6/2023, a patient payment of $179 was made. However,that payment was solely for the technical services (facility/radiology technician fees) that were rendered at the hospital and does not include the professional fee for the radiologist to interpret the exam (hence a separate billing statement from our company was issued to the patient).
Furthermore, A **************** Agreement was signed by the patient, acknowledging the following, I understand that many of the professionals who provide care to me in the hospital/facility are not employees or agents of the hospital/facility. These professionals may include my own physician, other physicians requested by my physician to participate in my care as well as emergency departments physicians, radiologists, pathologists, and anesthesiologists. As a result, I understand that these professionals will bill me for charges that are separate from those of the hospital/facility. Further,I acknowledge this contract means I personally have full financial responsibility for, and agree pay, all charges for the hospital/facility and of physicians rendering services not otherwise paid
Ive attached a copy of the signed **************** Agreement as well as the signed Waiver of Insurance form.

I am rejecting this response because: I personally talked to Centura health and ask what the total cash price would be to have the ultrasound at their hospital on a recorded line. They told me $179. They did not advise of any other costs. When I called them they told me to call you. When I called you ******* was unable to help in any way. This would be true except I specifically asked for all costs for the cash price. When someone answers how would I know they were lying to me. If I had known there would be other charges or was advised of that I would have cancelled and gone to another hospital. I thought I was getting services from the hospital that they employed and was never told otherwise when I specifically asked. This is pure fraud. The entire conversation was recorded by Centura Health. We cannot make determinations on our health care that are expensive when we are not told correct information by the people we are seeking service from. This is not a case of he said/she said it is recorded that was what was said.

I received multiple statements regarding and account so I paid it online as it dated to do so. They then said they were refunded me the money since I overpaid it the refund amount in my statements is $0. Ive called multiple times and everytime they say they left a message with the billing to have them give me a called back i havent received one call back. Good afternoon,
We sincerely apologize for the experience this patient received from our company. A review of our billing data shows there was an account balance of $155.71. A successful credit card payment for $77.85 was made on 1/13/2023. There were two attempted online credit card payments ($77.85 each) made on 1/27/2023 and 2/10/2023 but unfortunately, were both were declined. On 2/22/2023, a successful payment was made for $77.86, bringing the account to $0. On 3/9/2023, another successful online payment for $77.85 was made. Shortly after, our billing company discovered this was an over-payment and the credit card was fully refunded on 3/16/2023. Then on 6/4/2023,another successful online credit card payment was made for $155.71. Once again,our billing company realized there was an overpayment and refunded the credit card on 6/9/2023. Attached are both online credit card receipts and the subsequent refund receipts as well as both declined credit payments receipts.

I have showed screenshots of my account with my card number that you have on file with the amount 0 in the refund I hve called my card company and they say there has been no refund of ****** only the charge. If I have to go to court and file that way I will I have tried multiple times to call so that if I need to drive all the way to colorado to get this situation handled correctly I can. Like all the others here, my wife was sent to collections over bills we never received. We paid them over $900 in bills we DID receive, but they sent ** to collections over $62 in bills we NEVER received. This company has some serious billing issues that they need to address. A review of our billing data shows a patient balance of $64.19 (after insurance paid their portion of the services rendered). Our billing department sent a billing statement 11/14/2022 to the home address we have on file (******************************************************************; same address listed on the complaint). After no response to the statement, our billing department attempted to call the patient on 11/28/2022 & 12/27/2022, again there was no response from the patient. On 12/12/2022, our billing company sent a formal letter to the home address on file, notifying the patient we were unsuccessful in reaching out via mail and by phone, again no response. After these unsuccessful attempts to contact the patient, our billing department followed procedure and sent the account to our collection agency.

I am rejecting this response because: This is not true. You claim you sent mail and called. That's simply not true. I can understand if this was a single instance of being sent to collections after never receiving a bill, but it's not. Just ready EVERY SINGLE REVIEW of this company on BBB or google. EVERY review of this company says the same EXACT thing. "Sent to collections after having never received a bill". This is not a single incident. This is a standard practice for this company. A collection item was submitted by Colorado Imaging in Dec 2022 for $534. I have never received a bill from them and have no knowledge of what this is for. It is now appearing on my credit because it was submitted by ****************************. I called Colorado Imaging and asked for information regarding the item submitted to collection and they said they would send it to their review department and I would get a call. Dear ****************,
I understand your frustration with your billing for the ** imaging done at ***************. We are the radiology group that provides interpretation services for ***************. I have researched everything, and I do see that ** imaging was performed there on August 26, 2020 which is what triggered this billing process.
For some unknown reason, your demographic information did not contain an address or phone number so the account was placed with our up-front collection process which is designed to find that information. They were able to locate the address of ******************************************************************************************* along with the phone number ************ that you list on this complaint. The billing process then started their initial billing on December 2, 2020. The billing service reached you on December 15, 2020 and you stated that you did not have imaging done and that you would contact the facility. Our account with you was then placed on hold. We did not hear back from you on the validity of the claim of not having imaging done as we still show an imaging report. We resumed activity on the account in Sept of 2021 with calls and statements. This process continued through November of 2022 and now the account has been forwarded to legal for failure to pay the debt.
With your permission, I will upload documentation of your visit to Avista to support that imaging was performed on Aug 26th of 2020.
That puts us here with the BBB complaint. We are willing to work with you on this without legal assistance, however, you will need to respond to the billing. We do accept payment plans. Currently, *** placed the account on hold and hope to hear back from you by January 27th.Kind Regards,

I received calls and letters from ******************************** in the last year regarding a bill from Colorado Imaging Associates. After repeatedly requesting information regarding the collection item, I got a copy of a statement that showed a date of service of 8/26/2020 at Advent Health *************** in ********** Colorado. The date of service was a Saturday and showed that I had a brain scan and pelvic exam at this hospital. I have never had these scans nor have I ever been to that hospital as I live in *************** ** and this hospital is 41 miles from my home. I was employed and had good health insurance through my Employer. This is a totally fictitious charge and I told the *************** pursuing payment that this was not my bill. There are numerous complaints of this very same nature on the internet for Colorado Imaging Associates. Please investigate the business practices of this company. They served me with court documents and are taking me to court for payment of over $600. They are threatening to garnish my wages for payment. I contacted Douglas County Courthouse regarding the court date on the documents and there has been no Case # or Division # assigned and they do not at this time show that this date is reserved for my court appearance.Business Response

We apologize for your continued frustration regarding billing for the ** imaging done at *************************. As mentioned in the previous response, CT imaging was indeed performed there on August 26, 2020, which is what initially triggered this billing process. ************************* has a signed **************** Agreement as well as signed Patient Bill of Rights on file for services that were performed on that day of service. Thus, our billing company has recommended on several calls that you contact the hospital for further assistance with your complaint concerning the validity of the claim.
In our last response, we kindly asked for your permission to upload both signed documents to this BBB complaint. In the meantime, we agreed to put the collections account on hold. However, we did not receive a response and thus the account reverted back to active collections status.
Moving forward, we will need proof that this was not your CT imaging. In an effort to help both parties clear up this issue, would you please upload your drivers license so we can compare the signature to the signed hospital documents?Kind Regards,

Just been notified that my bill went to collections, I have never had an issue paying bills on time. I can't pay a bill that I never received. Just like all these other people, Colorado Imaging Associates just sent my bill to collections without even contacting me. I called them and their billing company said they called me back in February of last year 2021... I don't believe that one bit. I had a CT scan done on 2/28/2022 at St ********* in **********. I received the hospital bill and a bill from Colorado Imaging Associates in the amount of $61.71. That was paid in full in July 2022. On 11/22/2022, I received a call form a collections agency saying that I owed $392 to Colorado Imaging Associates ****** I never received a bill or phone call previously for this charge. *** told me that my insurance had denied the claim and that I was responsible. I called my insurance company (Anthem BCBS) and filed an appeal, which is still in process. Upon further discussion with my insurance company, I learned that they had previously denied the claim submitted by Colorado Imaging Associates twice: the first time on 3/4 2022 and the second time on 4/5/2022. My Explanation of Benefits shows that the claim was for $392 but the balance I owe is $0. Because the medical provider did not seek pre-authorization, and the insurance company saw the procedure as medically unnecessary, it was denied by the insurance company and determined that the charge is the responsibility of the medical provider. Further, the insurance agent let me know that it is against the contract between the medical provider and Anthem BCBS for *** to bill me for that balance, otherwise known as "Balance Billing". In the State of Colorado, this is also known as Surprise Billing and is illegal. I requested a statement from Colorado Imaging Associates and they were unable to provide one, stating that my account had been closed and sent to collections. On 12/19/2022, a third party person showed up at my parents resident (in a different city than where I live) trying to serve me papers for this charge of $392.On 12/20/2022, I have paid collections what they were asking for ($459.03 = $392 + interest) but ultimately, this charge breeches the contract between the medical provider and my insurance company. I would like a refund of $459.03 and to warn others of this malicious practice.Business Response

I am so sorry for the poor accounting on your claims. I have worked with the billing company account manager and she indicates that this was our error in how your account was handled. The second CT was denied and should have been written off and yet somehow that did not occur.
I apologize for this error and I have requested that they process a total refund to you for $459.03. I have also verified that this has been cancelled at the debt collection agency and that there are no outstanding legal issues or any notifications sent to credit bureaus. I have asked our billing account manager to look at her account processing processes to ensure that those performing these tasks do so with accuracy.
Again, I am truly sorry for the error and the mental anguish this has caused. Fraudulent medical bill from Colorado Imaging Associates. They sent me a bill and harassed me with predatory phone calls about $26 after they billed my insurance over $300, despite me never signing any form that I was consenting to them performing services during my ** visit. The forms I signed I read very clearly and the services I would receive would only be billed through Centura health. This is a money grab and a predatory practice. The doctor listed on this bill does not even practice in the state of Colorado. I have reviewed the account information for ********************** and I can assure him that this is not predatory or fraudulent billing. I have attached the signed authorization which clearly states in section 2 that there may be independent practitioners. Radiology studies are read by a licensed and fully trained radiologists who are not employees of the hospital and thus we bill independently.
The radiologist that interpreted his study does reside in ***** however he is licensed in Colorado to perform his work via teleradiology. He is fully trained and credentialed with the Centura facility where this study was performed. He is listed on our website as a group physician.
We billed his insurance and they applied $26.65 to his coinsurance. We followed our normal billing protocol.he got a bill on 10/14, 11/14 and a call on 11/28. He called on 11/29 and said he never authorized for our services,said he will contact his lawyer to fight this and requested a bill which we sent out on 11/29.
The charges are legitimate.
