We are a small pediatric office in ********* that sought accreditation as a Patient-Centered Medical Home (****) from the National Committee for Quality Assurance (****). This program was supposed to improve our workflow and patient care. According to the **** website, all of the ******** plans in ********* acknowledge the **** model. After going through a year-long process to get certified, spending thousands of dollars on the program, a consultant, and staff training, we found that none of the ******** plans recognized our accreditation. Despite reaching out numerous times and writing a letter to customer service, our concerns have been ignored. This program has been a waste of time and money for our clinic. We want to warn other small practices in *********: do not trust that you will receive any incentives for this program.

******************, NCQA is sorry to hear that your practice has chosen to not continue with NCQAs Patient-Centered Medical Home Recognition Program. We have checked with our internal teams and do not have records of receiving a letter. We do have records of phone calls to discuss the program and state financial resources. As an independent organization, NCQA neither recommends nor decides financial incentives, rewards or funding from payers or other supporting organizations. NCQAs website identifies supporting organizations and identified Louisiana Healthcare ****************** plan as a supporting organization that offered coaching and technical assistance to practices but was not identified as offering financial support. If you have additional questions or would like to resubmit your letter for a response, you can find NCQAs address and Customer Support information detailed at Contact Us - NCQA - *******************************************. Thank you.
