    This is to dispute the charge of $45 of office visit on 3/14/2024. Ref. Acct #P9137.
    As a complication arose after cataract surgery by Dr ****, my previous single vision turned into double after eyesight was restored. During post-op visits, Dr **** asked me to wait and see. Unfortunately, the diplopia did not go away. Although the post-op period was over, I should not be liable of the follow up costs because the delay was not my decision. Dr **** told me that he won’t charge me for the visit. When I explained this to the billing person, she did not listen.
    In addition, the statement shows a comprehensive eye exam and a refraction test. it’s my knowledge that refraction is included in comprehensive eye exam, so this could be double-billed. Despite the fee charged of a refraction test, I could not get the eyeglass prescription from the office for which I was sent back to my referring eye doctor. Hence, I need to pay another refraction fee.

    Refractions are not covered by medical insurance, only vision insurance, in which we do not participate.  Patient declined refraction and was billed in error.  The balance was adjusted off on 4/4/2024.  We do not participate with vision insurance and do not routinely do refractions in our office.  The doctor saw patient on 3/14/2024 and referred patient to another optometry office for a "spectacle correction, probably to include prisms."  We do not provide this service at our office, we are strictly a medical practice and do not perform vision procedures.  Patient was also informed at the time of his evaluation that he would still need glasses following cataract surgery, especially for near vision.  My office manager spoke to patient on 4/10 and explained about the referral to other office glasses for prisms but patient is confused.  We can not provide a refraction in our office as the patient does not want to pay for the refraction and we are a medical practice and this procedure is not covered under medical insurance.  This is a self pay procedure at our office.  We have attempted to call patient several times since, but voicemail is a "text now" and we have not been able to leave messages.
