      I visited the Georgia Pain and Spine **** clinic the last time on October 20, 2022 and was released from the clinic per my request. I have been a patient at this office for several years. At the time of my last visit, I requested in person that my medical records be sent to my new physician at a new doctors office in ************, **. They assured me that my records would be sent. My new doctors information was provided to the Georgia Pain and Spine **** on October 20,2022. However my records have not been sent to my new doctors office, North Georgia *********** in ************. I can not begin treatment or even be seen at my new doctors office until they receive my medical records from ** Pain and Spine ****. I have called the records department at Georgia Pain and Spine **** numerous times since October 20, *********************************************************************** transfer my records. No one has returned any of my phone calls and messages. I have also sent a written request to transfer my records, which no one has responded to. No one ever answers the extension number 516 which is the records office at ************, the phone number for Georgia Pain and Spine ****. I want this office (Georgia Pain and Spine care / Pain ****, LLC) to forward all of my medical records like they told me they would on my last visit there on October 20, 2022. I need this done in a timely manner so that I can continue to receive the medication that I need.
