    **************** office continues to forward bills in the amount of $25.00 concerning a missed medical appointment from May, 18, 2022. The initial bill arrived at a relatives home in April, 2024. The name on the bill is *************************. My middle name is ******, however, I have never used my middle name for any business, medical, or personal related matters. The initial bill was sent to mailing address I resided at for less than 2 months after I relocated to ******* 2 years later. I do not have any written notifications, welcome letters, and or signed off on any written notification stating that I was aware **************** practiced charged a fee for no show appointments. This is nothing on their website site stating the aforementioned either. I am writting to request a resolution to this matter. It is an unfair business practice and appears to be an opportunity the practice is using to exploit money from me 2 years later without prior knowledge and or warning in writing, and or in the form of a voicemail. I have reached out and have been unsuccessful in my efforts to speak to someone, I even have been hung up on several times. As of this date, I have paid no money to this practice. I have uploaded an email I found about the scheduled and missed appointment. Neither mentions anything about a no show fee. I am also requesting that no bills be sent to the address they have on file. I have resided in ************* for more than 1 year.

    The no show fee policy is posted in my office in multiple locations and is also in our text and verbal communications with patients when confirming appointments. With that being said, we will write off this patient's $25.00 fee for not keeping her appointment and not notifying us.
