    I had an appointment for knee treatment and injections. They gave me a shot that infected my knee and cause me pain and more problems with the same knee. I had to make a loan through care credit for $5800.00 that I now have to pay back. I reported this to the Nurse that gave me the shots that was causing me so much pain and problems. Once I report this to the center they only refunded me the amount of $182.62. I would at least like half $5800.00 refunded back to me. I wanting a refund $2900.00

    Recently you sent me copies of the agreement I signed with pc medical center. In the papers was one I never seen before,

    the one next to the last page. 
    apparently added recently.

    On 11/12/2024 the patient chose to enroll in a treatment program to address her knee pain. At this time, she signed the following documents:

    1.       Office / patient agreements

    2.       Refund calculation agreement

    3.       To be delivered

    4.       Orthotic delivery form

    5.       Financing document for Care Credit

    We also have the signed Request and Informed Consent for Intramuscular Joint Injections dated 11/18/24 and 11/26/24, as well as the Informed Consent for Regenerative Medicine signed 12/6/24.

    All these forms are reviewed with the patient in detail prior to them signing and receiving injections.

    We are more than willing to send you copies of these consents and all financial agreements; however, I do not see that we have a PHI release on file. I am confident that once you see the degree to which we strive to elevate our patients quality of care and overall satisfaction, it will be evident that we have gone above and beyond to help this patient in every way possible.

    Ms. ***** was sent a refund check in the amount of ******, and a refund to her Care Credit account in the amount of 102.08.
