    On April 3rd 2023 I (*******************************) used the services of Community Ambulance for transport to a medical center in ********* **. The service nor the cost is of issue. The issue is that my insurance company applied a discount of $127.20 and paid the remainder of the bill in full. I have since received invoices from Community Ambulance stating that I owe the $127.20 that my insurance company states is a discount applied to my claim. Upon speaking with my insurance company, they stated that Community Ambulance is known to bill customer for the discounted amount, AKA Balance Billing, as they claim the discount does not apply due to state laws. But my insurance company, Cigna, states that they must accept the discount as it's federal law. and now state law in both ** (were I had the service and AZ where I reside). ***** has opened a claim dispute and is contacting Community Ambulance. But I am still getting bills from Community Ambulance. The lasted one dated 8/18/2023 included ****ATTENTION: PRECOLLECTION NOTICE****. I would like them to be put on notice to stop and be put not notice for this practice of Balance Billing.Date of Service: 04/03/2023; Provider: Community Ambulance; Trip Number: 150-73323442-00; Patient Name: *******************************; Account Number: **************.

    Balance was zeroed out.  Cigna had applied a 10% prompt pay discount to the account despite not meeting the prompt pay criteria, hence the confusion of the balance owed.  Community Ambulance wrote off balance vs pursuing through collections.

    The balance was recalled and written off last year (see attached).  I've reached out to ********* as to how the balance remains outstanding and to please write-off.

    I was in ********* this last December 2022, and took suddenly Ill( later diagnosed with Double pneumonia) an had a great trouble breathing, and paramedics were summoned to my room, where after evaluation- an Ambulance (Community Ambulance) was summoned. I was put on oxygen and transported to the closest emergency care facility, In January I started receiving bills for the ride, noting that my insurance denied the Bill because it was billed as just basic transportation - and did not include the appropriate coding to include oxygen of which I was in need of and provided!! If this code was to have been included the bill would have been paid!I have called every month once to twice and have told their customer service of this, improper charge, and coding issue. I have been assured this would be corrected. To date it was refiled and billed but with the same improper billing, I have called asked for a return call by management, received finally a call back in May of which I was told this claim was in dispute but it would be taken care of correctly To Date, It has not been taken care of thus far, nor has it been re- billed to insurance.. I cannot be expected to pay for this out of pocket, when I have great insurance and if this would be correctly billed it would be covered!! The business has not contacted me again yet again as requested and nor have they re- billed correctly!

    I just saw this email in my spam folder.  My apologies for delay in response.  I have reached out to our billing company to get the status of the invoice and to ask why it was coded the way that it was.  Will report back with a resolution.

    **All actions and calls listened below taken by my wife, *******************************. 03/31/22-Letter dated from Community Ambulance for trip number 150-87398788-00 and balance $1028.10 due to not meeting the deductible. Letter states that discounted rate of $925.29 if received by April 30, 2022. 04/28/22-Attempted to pay discounted amount online, but website only had full balance with no option to change. Called customer care ************ to ask for help before payment submitted and the rep advised they would make an adjustment and issue a refund of $102.81. 06/20/22-Followed up on the refund, was told nothing on file. The person made the adjustment and it would take about 2 weeks to process refund. 06/23/22-Adjustment posted. 06/24/22-Refund review started. 07/18/22-Follow up call, no change to status. 09/07/22-Follow up call, advised that there are 2 levels of approval. One level completed, waiting on second before being refunded. 12/01/22-Follow up call. Was advised that the ticket was closed and paid for by insurance, which does not make sense - we were billed because we did not meet the deductible. Per the rep, there was no open ticket for my refund. The rep explained that she would have to open a new refund ticket. Was told I would receive a call back from a supervisor within ***** hours. 12/07/22-Have received no call from the company.

    Community Ambulance Service for my Husband *********************** on January 28, 2022 Trip # ***-23752786-00 Amount I Paid $329.12 ****************************************************** ************** FAX 702-920-554www.MyEmsAccount.com Desired ******************* and United Health paid and told me that they did so and I did NOT OWE this amount. I have called this service many times since May I have cancelled check I am getting run around etc. No supervisor will speak to me or given name am speaking to *****. Told they r investigating check is in the mail etc.

    Received complaint on 9/20. Investigated and found that this was a round-trip type transport to and from a procedure. Trip from facility to Dr was paid for by facility. Return trip was billed to *************** Health who paid but later rescinded the payment denying the claim. **** was then ent to patient rather than facility. Facility is now being billed for return trip and I have immediately requested a refund be processed for patient. This process is done through a bank bill-pay system but will expedite.
