    I had dental services that were performed on July 11, 2022 at Bethlehem Town Family Dental with Dr. Henry H DDS. I had confirmed prior to any appointment or procedure that this provider and office was in-network with ****** ****** though the Find a Dentist online search tool as well as through the message and response on the online portal. I am the subscriber of the plan with the ID number: ************ The Explanation of Benefits was completed for an in-network dental practice with the dental claim number ************. However, Bethlehem Town Family Dental with Dr. H**** is claiming they are an out-of-network practice for ****** ******. Upon completion of my visit, I paid $729 for the whole visit. The dental office claimed that I would need to pay upfront the whole cost for my specific plan and would later be reimbursed from my insurance when they filed the claim. It was my understanding from the response and communication with ****** ****** that they were an in-network provider and my ****** plan would reimburse me for my visit. I would have never gone to this dental provider and practice if I was told they were out-of-network. After multiple communications with both Bethlehem Town Family Dental and ****** ******, they are both still claiming that they are correct. I have only been given the money that ****** ****** sent to Bethlehem Town Family Dental but I should have only paid a total of $68.20, instead of a total of 587.20. I am concerned that this may be happening to other patients that are being told to pay upfront and over paying for their services. ****** ****** shows that a contract is still in place for my preferred provider plus network. If Bethlehem Town Family Dental did not appropriately end their contract with ****** ****** then they should take responsibility for and reimburse me the appropriate amount and continue with actually ending the contract with ****** ******.
