    I am writing to express my deep frustration and disappointment with the recurring issues I have faced at HVRA in ******. Despite being a returning patient there, as well as their sister facilities, and confirming multiple times that they are in-network, I was shocked to be presented an out of pocket cost of $400 for a mammogram that should have been covered by my plan and the law. After providing a reference number from my insurance company and reiterating my coverage, I was assured that my out-of-pocket cost would be $40 for the visit, US and mammogram. However, upon arriving at the appointment two weeks later, I was once again confronted with an amount due exceeding $400. I insisted that they check their records and reference number, which ultimately led to me paying the correct amount of $40 for the scan and ultrasound. A week or so after the appointment, I received a bill for over $1000, clearly an attempt to extract more money from both me and my insurance company after their failed attempts to incorrectly and illegally charge me more beforehand. This is not the first time I have encountered such deceptive practices at this facility. Previously, when I needed an MRI, I was pressured to schedule the appointment before an authorization was obtained which resulted in the insurance company coming after me for more month. Furthermore, I later discovered that they had lied about having obtained the pre-authorization a second time, potentially jeopardizing my coverage again. Dealing with healthcare in ******* is corrupt enough without having to navigate such unethical and deceitful practices every time I have an appointment or scan. Additionally, I had an MRI scheduled for today which I had taken time off of work for only for them to contact me yesterday stating they do not have the open MRI i had confirmed that they indeed had at the time of scheduling. They were supposed to r/s me for early next week but never replied and now I am screwed since auth expires may 15.
