      I was referred to Firehouse Diabetes and Endocrine Center for follow up for the presence of thyroid nodules on 10/28/2021 and I provided my insurance card to the front desk person took the information to be entered it into the system, I was seen by a clinician and was told there was no treatment needed at this time, just yearly follow up and was given an appointment for 10/26/2022 one year later. I received a notice about 3-4 weeks later stating my insurance denied my claim. I noted the info was under my previous insurance coverage and not the one I had provided. I had not been covered by that insurance for at least two + years. I stopped by a couple of days later in person and provided my card information again and told them of this problem, the front desk person stated "I'm not sure why that would happen" and appeared to correct it for resubmission. I assumed this would take care of the issue. In mid December I rec'd a notice from Firehouse again that they had submitted my insurance twice and were closing the clinic on 12/17/2021 and if payment was not rec'd prior to closing they would send to to collections. I was provided a form to submit on my own and the information was all incorrect, ******************************** was not my PCP any longer and did NOT place the referral, also it lists Samaritan Health as my insurance which I had corrected already twice to no avail. This document is provided and signed 12/16/2021 just one day prior to the scheduled closing of the clinic which I had only rec'd notice of just prior to getting this notice. I called the number provided and left a detailed message. We spoke on Dec 20th and I was informed that her phone was being cut off in just two days and she could not stop the collection action. I have very good credit rating and didn't want it to be a huge hassle so I paid the bill. It was $477.00. The woman to whom I spoke said she would try to resubmit the insurance info ( I provided my correct insurance info for now the third time) her phone would be turned off in just a couple of days and could not help me further. I did receive a statement several months later, probably April 2022 stating they had paid the majority of the bill. So now I don't know how to recover the money I paid in error...THEIR error. I did call the number from the website but the person who answered said they only have patient records and no billing information. She suggested making a complaint with the BBB, Thank you for your help I will upload 1 documents, unable to get the other document I have to load...
