    I request your intermediation so The ORTHOPEDIC GROUP institution sends me a copy of the medical history and the CDs where the x-ray exams that I had around March 2016 . In the offices of this institution, at Jefferson. At the request of Dr. F*****, ****** B. MD, Please. 
    I will describe the facts: 
    I, ***** ******* ****** **** ******* Address: *** * * ** ** ********* ** ***** and telephone: ###-###-####. Email: ***********************. On June 27, 2022, I emailed The ORTHOPEDIC GROUP managers from the offices located in Charleroi, Mt. Lebanon, Rostraver Township and Uniontown. (EXHIBIT 1).In this email I requested a copy of the CDs that The ORTHOPEDIC GROUP gave me, which included the X-rays that were made at the request of Dr. Florez, G***** ** MD at this institution. I also attached part of the medical history as an attachment, where there are more details about my medical history. (EXHIBIT 1). 
    Seeing that I do not receive any email response from this institution. On July 2, 2022, I sent a second email to The ORTHOPEDIC GROUP manager of the charleroy offices, Mrs. ****** ******** ******************************* Where again I request you to send me a copy of the CDs of the x-rays, made at the request of Dr. F****** ****** *. MD. ( EXHIBIT 2 ). 
    Why I do not receive any response from this institution. I request your intermediation so The ORTHOPEDIC GROUP can give me a copy of my medical history and the CDs containing my x-ray exams. 
    As I have basic knowledge of the English language. I also attach this petition in Spanish.
