      I was asked a series of questions as a new patient and a few I found were very disturbing. I have never had a doctors office ask for my SS number over the phone and second what does the **** have to do with a fully insured employee of the insure company, what is my ethnicity!? I was told i couldn't be a patient if i didn't want to tell my RACE!
