      Date of service was 7/14/22. I was forced to make a copayment of ***** that I KNEW was not required. The claim paid on 8/30 and no copayment was required. The billing office refuses to return my ***** even after a rep from my insurance called them. They plan to hold onto my *****, waiting for a copay to be required. My insurance does not require a copay when treated for eye disease. For some reason those in billing do not understand English even when all other claims have required zero copay.

      This is in response to complaint number ********. The complaint states that ******************************* asked for a refund of ?35 for a copayment for date of service July 14, 2022. According to the patient's insurer. A copayment of ?35 was due at time of service. After the phone call with the ****************** a refund check was mailed out to ****************** and was cashed by her on October 26 2022.
