    On 8/9/2023, my 7 year old son had an appointment with ******************** for a neuropsychological assessment as his speech therapist and occupational therapist both suspected he has Autism. We completed the assessment that day, I paid 500$ and they had my insurance information and I was told by the front office staff they had contacted my insurance. I have emailed them from their website around the first of November.***************************** emailed back 11/8 stating that she could email or mail it. I wrote back the same day asking her to email it. That is the only correspondence I received. I emailed again trying to get this evaluation on 11/22/24 and 12/8/2023. I left voicemails on 10/25/23, 11/7/2023, 11/28/23, twice on 12/5, and twice on 12/8/23. My husband has called and left a message 1/23/24 and 1/24/24. He stopped by their office 1/23/24 and 1/24/24 but the office was locked and nobody answered to the door. As of today 1/24/24, our insurance has not been billed. My son needs services but we need this evaluation to apply for ******** TEFRA. We also paid 500 for a service and we havent received what we paid for.

    I received a letter in November, 2022 from *********************************** stating that a billing error was recently discovered and I owed a balance of $575 for an exam that was done 5/1/2017. The letter stated "if you need to start a payment plan, contact our office for arrangements". I attempted to contact the office several times and the phone number on the letter, *************) with no answer and no way to leave a message. When I located the clinic using the internet, the website redirected me to another IP address. I emailed the clinic twice, finally receiving a response on 12/12/22 by a ***************************** who stated that the bill could only be paid using a Credit Card Authorization form. I declined to provide my cc information, instead sent a payment of $25 dollars by cashiers check and registered mail. ***************************** sent several subsequent emails to my personal email address (using my maiden name which has not been accurate for 22 years and is suspicious how she obtained that information) which were both threatening and bullying in nature, stating legal action in the form of small claims court and late payment fees/interest if I do not provide my cc information. I have asked Pediatric Neuropsychological Services staff many times for the financial agreement and payment policy, as well as the payment plan information that was noted in the first letter. None of those requests have been honored. I am suspicious of the clinic and the bullying/threatening nature of my interactions with staff.
