    I left a form for disability insurance with them on 9-16-22. They charged me 30 dollars to fill it out and said it would be ready in 10 days. After 2 weeks I returned and it was still not completed. I made several calls before someone told me that they would not fill the form out until after I had surgery. I went to the office, asked for the uncompleted form back on October 5th and requested a refund of the 30 dollars. I called the office November 8th and spoke with ***** who said she had requested it on October 5th but had no idea when I may receive it. Two months is completely unreasonable to receive the refund and if I owed them 30 dollars for 2 months they would have reported me to the credit bureau!
