    Promptly address several issues w/ both ******* childrens medical records, which I have repeatedly brought to your attention. Despite my efforts to resolve these matters directly w/ your office, there has been an ongoing failure to provide the necessary corrections, & etc. Over the past 3 months, I have requested copies of medical records from your office, yet I have not received them. Furthermore, I notified your office of inaccuracies in my medical report to Mrs. ***** & many front desk clerks & specifically requested a letter. This letter is essential to verify my medical history for negotiations concerning a nail infection I suffered. Despite assurances from our office that these corrections would be made, the records remain incorrect, & the requested letter has not been provided. Additionally, I have found numerous false & inaccurate entries not only in my records but also in my childrens medical records, raising significant concerns about the quality and reliability of our medical information as maintained by Huntsville Family Health Center. These inaccuracies, delays, & the failure to provide the requested letter have caused me considerable inconvenience & have hindered my ability to proceed with necessary negotiations. This serves as a final demand for the following.Correction of All False &Inaccurate Information, Provision of Requested Medical Records, Provision of the Requested Letter, Compensation for Damages: Due to the delay in correcting the records, the failure to provide the requested letter, the inconvenience caused, emotional distress, and the need for deterrence and accountability, I am seeking compensation in the amount of $95,000. Failure to comply with these demands will leave me w/ no choice but to pursue all but not limited to, filing a formal complaint to: *************** of Health and Human Services,(OCR), Alabama State Board of Medical Examiners, HRSA) ., Centers for ******************* Services., pursuing legal if deemed necessary.

    I wish to proceed with litigation. Huntsville Family Health Center has failed to respond adequately to my concerns regarding the false and inaccurate information in my medical report and those of my four children. This demonstrates an ongoing issue within their organization that affects the integrity of patient records.


    As of today, October 16, 2024, I have not received the requested records, despite making three requests over the past several months. The statement made by the company that my request was processed on 8/12/2024 is not consistent with my experience, as I have yet to receive the records. Furthermore, the inaccuracies in my records have directly hindered my negotiations in other legal matters. Once my attorney received my HIPAA records, it became evident that the records provided by Huntsville Family Health Center were false and/or inaccurate, complicating my case and delaying potential resolutions.


    I am prepared to escalate this matter further if necessary.

    I am rejecting this response because:

    Thank you for the opportunity to clarify the situation. I appreciate the time and effort that Huntsville Family Health Center has taken to address my concerns thus far. I believe that there may be some confusion regarding my records request, and I hope we can resolve this together.


    The statement from Huntsville Family Health Center indicating that I received my records on August 12, 2024, is not accurate, much like the inaccuracies I have experienced with my medical records. To clarify, I have made multiple requests for my records, beginning in March 2024, continuing in April 2024, and again in August 2024. Despite these efforts, I have yet to receive the complete records.


    On August 12, 2024, I did receive a printout from Mrs. ****** releasing me to return to work. During this visit, she attempted to print my records but was unsuccessful, and I was directed to the front desk for further assistance. At the front desk, I was informed that I needed to fill out another form for Mrs. ***** I explained that I had already submitted this form several times but had not yet received my records. The front desk representative was unable to explain the delay.


    A week later, I returned to the office and spoke with a gentleman who had assisted me with my initial request in April. He expressed surprise that I had still not received my records, saying, You still havent gotten your records? When I confirmed that I had not, he replied, I dont know what to tell youI turned it in.


    Additionally, we have yet to address the inaccuracies in my medical records and those of my children, which remains a major concern for me. It is crucial that these inaccuracies are corrected to ensure accurate documentation and proper care.


    Finally, the delay in providing my records and the inaccuracies present have significantly hindered my ongoing legal matters. Having accurate and timely records is essential for me to proceed with these negotiations.
