      On or about Sept. 20th, 2022, Ive come to your store with my Medical Doctors prescription for diabetic shoes and custom made shoes inserts and I provided your office with the prescription and my medical insurance card and information.I had to go through several failed attempts to receive the shoes and the inserts because of different excuses such as:* First time, the excuse which was told to me by the employee on the front desk the shoes company doesnt have the shoes model that youve chosen and you have to wait tell they have it.* Second time Ive received a phone call to pick the shoes, when I went,the employee refused to give them to me claiming that, the technician is not their and she didnt know when or if he will come that week.The insurance coverage for such item is renewable yearly and you have failed to file as they require you to do.Therefore, Ive lost my insurance coverage for the year 2022 for that item.When I received your request for me to pay $210.18 and your front desk employee told my wife that she will send this as debt to the credit bureau in three days.My wife had to pay it to avoid a derogatory note on my file to keep my excellent credit history clear.After my wife paid the total amount shown in the invoice, surprisingly, your employee informed me, that she doesnt have neither the shoes nor the custom inserts.

      GOOD AFTERNOON!

      THIS PT WAS INFORMED THAT WE HAD TO GET PAPERWORK TO FILE THE INSURACE CLAIM FROM ************** BEFORE WE EVEN PLACE THE ****** THIS DOES TAKE TIME BECAUSE DOCTOR'S HAVE A LOT OF PATIENTS AND ARE BUSY. WHEN WE PLACED HIS ***** TO BE SHIPPED/MADE WAS PLACED ON 11/18/2022. THIS ***** WAS DELIVERED TO OUR FACILITY ON 12/16/2022. WE CALLED THE PATIENT TO SCHEDULE A DELIVERY APPOINTMENT WHICH WAS 12/20/2023 AT 9:30 AM. WHEN HE NO CALLED/NO SHOWED. WE TRIED MULTIPLE TIMES TO SCHEDULE HIM BUT EVERYTIME WAS A NO CALL/NO SHOW.  WHEN WE FINALLY HEARD FROM HIM, HIS INSURACE HAD CHANGED TO UNITED HEALTHCARE BUT HE WANTED US TO BILL HUMANA WHICH HE HAS NO MORE. WE TOLD HIM THAT WE FILE INSURANCE ONCES ITEMS HAVE BEEN DELIVERED TO THE PATIENT AND THAT WE COULD NOT DO THAT LEGALLY. HE GOT ANGRY TOLD US NOT TO WORRY ABOUT IT AND STORMED OUT OF OUR OFFICE. WE TRIED TO CALL MULTIPLE TIMES AND HAVE LOGGED IT. SINCE THE INSERTS THAT THE PATIENT WAS SUPPOSED TO GET WERE CUSTOM MADE THEY CANNOT BE USED ON ANYONE ELSE PER THE ***** PATIENTS ALSO SIGN A FORM STATING THAT THEY ARE FINANCIALLY RESPONSIBLE FOR ANY REASON. PATIENT HAS GOTTEN INSURANCE TO CALL US AND HAS BEEN EXTREMELY RUDE. AND ABSOLUTELY NONE OF OUR PAPERWORK TRYING TO COLLECT PAYMENT ONLY GIVE PATIENTS "3 DAYS" TO RESPOND.  WHEN THE PATIENTS WIFE CAME IN TO "PICK UP" HIS ***** I HAD TO TELL HER THAT IT HAS ALMOST BEEN 1 YEAR SINCE THE ***** WAS PLACED WE NO LONGER HAVE THE ***** AND I EXPLAINED WHY AND SHE JUST CONTINUED TO TRY TO START SOMETHING AND SHE EVEN TOLD MY BOSS THAT COME UPFRONT THAT SHE WAS NOT ALLOWED TO TALK.
