    I was referred to Champion Sports Medicine from ****** Health in ********. I have been homeless with no income. Staying at *********************** when I was referred for physical therapy due to arthritis in my back. I had a date of service with Champion Sports Medicine in ******** on 11/8/24. ******************* referred me per my request needing an "in-network" physical therapist compatible with my insurance coverage. I was referred to Champion Sports Medicine in Vestavia. I called them prior to my appointment asking them about my insurance to make sure I would not owe anything and that the visit would be covered 100%. I was repeatedly assured by the lady on the phone (*******) that I had like thirty free physical therapy visits on my insurance coverage to use before the end of the year and would be totally free to me. I only had one visit with them which was on 11/8. I later (after the appointment) find out from my insurance based on the claim for that physical therapy visit which is listed as ****************** that the claim was denied. My insurance was billed for $284.00 but because it was denied, it is showing that I owe $284.00 which is not right or fair according to what I was told over the phone by Champion Sports Medicine. I thoroughly checked before making and going to appointment that I would not owe anything. They assured me the visit was covered 100% on my insurance which is why I made the appointment.

    Hello,

    Thank you for bringing this concern to our attention. We have looked at the the verification of benefits again which lead us to contacting our billing office. Our billing office is offsite. They have informed us that the initial claim was denied, no bill was generated. The billing office has resubmitted the claim and it is still pending in status. We do not have record of the patient receiving a bill. There is not a balance listed on the individual's account. We have asked the billing office to reach out to the patient individually to help explain the matter.     I am rejecting this response because: I don't understand the reply the business. ******* specifically told me when I called to schedule my first and only visit, there would be absolutely no charge to me based on my insurance coverage. ************************ is my insurance. I have a balance with them for this visit which totally conflicts what ******* told me on the phone. I am requesting for this balance to be cleared based on what she told me.
