  9/14/2023 I went and got an allergy test performed at Arizona allergy associates. I paid my $20 co pay this visit and was told I would be contacted with the results. 9/20/2023 I was contacted with the results and they told me they checked with my insurance for the cost. They sent me a email of the cost break down. I did not respond to the email. 9/27/2023 they contacted me telling me to get a more accurate cost I need to fill out the consent form. Which I filled out. I checked the box please contact me with a quote before making any extract. I received a call from **** which we talked about the cost and the process and I told her I could not commit to the program cause I had to do it for minimum 2 years and I wont be in Arizona long enough to complete it. And she said ok and that was it no contact from them for the next 2 months. 12/1 I recieved a bill from them for $340.40 For an extract that according to the bill was made 10/26/2023. I contacted them and they said I filled out the consent form so they made the extract. I told them I did not accept the terms when I was contacted. I was told well we made the extract so you owe us money.My wife has been dealing with ***** in billing trying to get this resolved to no end. On 1/5/2024 we recieved another bill but this time for $477.02. So we contacted them and was told by a girl named **** that they are charging us still for making extract for the program. They are also charging my insurance for these injections that I have not recieved a single time. 2/16/2024 we recieved a letter saying we have ************************** we are getting sent to collections.2/20/2024 my wife talked to a woman named ******* and she said we are bound now to have to pay for the extracts they will continue to make because of a consent form that is marked for them to contact me before making the extract. And they were sending us to collections. So my wife paid ***** to stop it going to collections. I have only been here once
