      My son, ******************* started physical therapy on 01/02/2024. I have been experiencing issues with Therapy Tree's billing department for all visits since then. I did my diligence to confirm with Cigna that this provider is in-network and confirmed my benefits. Therapy Tree also confirmed coverage with Cigna prior to the start of the appts. Since the beginning, I have been paying a copay of $35 per visit. Based off the processed claims through Cigna, my cost-share should have been $6.98. I am due a refund, however Therapy Tree denies this as correct and refuses reimbursement back to me and also has not sent in any corrective claims to show a $35 copay as they believe it should be. On 04/15/2024, I contacted ***** and they confirmed no claims were denied and each show correctly processed at a cost-share to me of $6.98. Therapy tree has only submitted the following dates of services and these are the copay based of the completed EOBs through Cigna:01/17/2024 - $14.48 02/06/2024 - $35 02/13/2024 - $6.98 03/26/2024 - $6.98 04/02/2024 - $6.98 04/09/2024 - $6.98 04/23/2024 - $6.98 05/07/2024 - $6.98 Therapy tree also has not submitted the following dates of services for processing through Cigna:01/02/2024 01/09/2024 01/16/2024 01/23/2024 01/30/2024 02/20/2024 02/27/2024 03/05/2024 03/12/2024 03/19/2024 05/21/2024 Note, we have been seen by the same physical therapist, ***************, for ALL appts. Only on 01/17/24, were we seen by ******************* for occupational eval. Therapy Tree claims we've been seen by an out of network provider by the name of ****/***** and is why they are not submitting the claims I request a full investigation to have the claims properly submitted and processed. I request full reimbursement for any overcharges I have based of my EOBs and/or corrective claims sent to Cigna asap in order to apply to my max-out-of-pocket correctly. Attached are EOBs, receipts, messages to Therapy Tree and Cigna.

      I have repeatedly contacted this company to ask for the supervisor to contact me back regarding a letter from the therapist that evaluted my child and also was completing speech groups for my children. Both therapist had conflicting views on the method to help emotional disregulation for my kids and the manager was supposed to contact me back a few times and hasn't. I've emailed, I've had front desk give messages, I've texted. I need help.
