    I contacted **** ************* and was told to email her directly. She agreed to fill out paperwork but did not do it correctly. I emailed her and only learned later that the email was not confidential. I left several messages and emails asking Yeon Park to call me directly to protect my privacy. She instead forced me to speak with her staff who kept passing me off to at least two other staff members **** and Q. which I refused to do. ****** spoke with you in person for the reason of my visits during my appointments. I see you didn't take notes so perhaps you forgot. I need the paperwork filled out correctly as described in my email and re emailed to me today.
