      I scheduled an appointment to be seen on Monday August 26, 2024 at 8:00am and received an email for that appointment. I took time off of work and drove an hour to the office who informed me when I got there, the doctor was not there and they did not have me on the schedule. I was offered no apology or rectification for this other than to schedule another appointment. I arrived for my scheduled appointment at 8:40am on Tuesday September 3rd. I waited one hour after being told the doctor was on time. I was seen at 9:56am. The doctor completed the medical form I needed for my potential future employer, and the medical assistant told me the front office needed to make a copy and after that I was good to go. I gave the front office the form to copy and she handed me back a medial release form and consent. I advised her that I would be providing the form to the company myself. As a government entity, there was no information I was given to provide to the doctor. Reading the form the front office gave me, it was requesting a signature to authorize the release. I repeated again, that I would be sending the documentation to them myself and that the office did not need to send anything. I also advised that I did not have the information that she was requesting me to write. She then yelled at me and stated "I have a job to do and this is the information we need for the ***** release." I repeated that I would be sending it, not them, and that I was not signing an authorization to release information because I did not authorize anyone else to have this personal medical information. "She then said this is our policy and we have to have this filled out." They would not allow me to leave without completing that form authorizing the release of my medical information even though I had the completed form to provide to my future employer. Another MA stated "just write down that he's refusing" not addressing me directly, after I had completed the form with my address and phone number.
