      I booked an appointment with **** office I told the appointment setter I was not a new patient and that i had been there before she ignored me and set the appointment. I get to the office and advise I am not a new new patient the woman ignores me and hands me new patient papers.I turn the papers in and INSIST I am not a new patient that I had been there before for the same issue. The lady finally asks me a couole questions and pulls up my profile. She acknowledged that i had been there before under my prior last name. I got a bill in the mail from the office charging me for a "new patient" visit. I am not a new patient. I was at this office multiple times under ******************************** which is my birth name. I tried so many times to tell them and they would not listen to me. I WANT MY BILL FOR ****** REMOVED and to never do business with these people again! ?? They are stealing via clerical bull c*** AND medical insurance fruad because they billed my insurance for new patient services when i was not a new patient. ??

      Phoenix family medical Center sent me a text message for a $280 bill. The text had the link MYPROVIDERLINK.COM I paid it with my HSA card through there and it said "error, try again", so I did three times. A few days later I looked at my HSA account and it had 3 transaction taking $280 each time. I contacted them and they said they hadn't gotten anything from me. I called my HSA bank and they said Phoenix Family Medical Clinic accepted 3 times for $280 each. I want my card reimbursed.
