    my Husband william sloat passed April 24,2024 when we did the paperwork i stated i wanted his clips from his lung removal he had in 2000 that was his only surgery well when we picked him up on the 17th of may we did not get my husband we got some one else because they gave me a hip replacement and knee replacement and i have a Dr, later stating he never had a hip replacement and stating also has no records of a knee replacement she just did x-rays on his hip a year before he passed and he never under went surgery for a hip replacement with in the year and they gave me a file from angel valley funeral home that was to be checked off by crematory operator and he did nothing i have the file they sent and they still stating its my husband ashes

    Prepaid Cremation Policy with Angel Valley Funeral Home. Husband, Jeffrey R. Saylor, died 5/24/2023. Death Certificate red flagged by Medical Examiner's Office, 5/28/23. Angel Valley received Death Cert. 6/5/23. I picked it up on 6/8/23; later that day, I discovered the wrong death date. Called Angel Valley to report the problem. Asked if corrected Death Cert. could be mailed to me. Yes, only by Certified Mail, $35 charge. On 6/13/23, I was contacted by *** ****, Risk Management, St. Mary's Hospital. Working on resolving problem. St. Mary's will cover expenses for six Death Cert. ($20/ea.) and Cert. Mail ($35). On 6/13/23 I paid Angel Valley $155, Visa Card. They will notify me of the delivery date. Not notified. On 6/21, I received the Death Certificates in a Priority Mail Flat Rate Envelope, $9.65 postage. I called Angel Valley, could not speak to Elise, Funeral Dir., spoke with Annalise. Complained about the delivery since I had paid for Cert. Mail. Got a runaround, it was insured, had tracking, but it was not what I had paid for. I needed a refund of $25.35. Not going to happen. I notified Ann Ross, St. Mary's, since they are reimbursing me for expenses. I realize this may seem to be a petty complaint, but how many other people has this happened to who haven't bothered to report it because it is too much trouble? I was promised a particular service, I paid for that service, I did not receive that service. I expect a refund to be paid to me or to St. Mary's Hospital. I might add, this is the only problem I have encountered with Angel Valley Funeral Home in my dealings with them.
