      I have showed up to my appointment and on numerous occasions they have Turned me away due to changing my primary care provider without any prior notice what so ever. This has happened on more than 3 occasions where in the terms and conditions I was supposed to be informed prior to the change and it would be okay but considering Everytime I've showed up to my appointment I've had to turn around and go home due to this situation is absolutely some form of mal practice or discrimination or something. I have several different medical conditions that I NEED help with tons of Labs needing to be done referrals etc. and each time this happens it sends me right back to the starting point and that is preventing me from receiving efficient care! I've been trying to get the paper work for **** filled out by my primary and I am unable to do that because 1. They keep changing the primary on me or 2. When I do see the new primary he states he doesn't know me well enough to sign my paperwork which is another form of malpractice/discrimination considering he has access to my medical records/notes given by the previous doctors . I'm severely disabled and I need care with things I'm unable to do myself and I am prevented from receiving care because I am not able to get the **** paperwork completed. I will be speaking to a lawyer about all this unecessary troubles the Doctors are causing me along with IEHP dual choice. I am left with no other option because my life is at risk and Everytime I get hit with this same situation it puts me even more at risk.
