    At a previous dental office visit, I received a periodontal maintenance cleaning. During a recent visit, the hygienist performing my cleaning stated I would receive a routine cleaning because I did not need a periodontal maintenance cleaning. After receiving a routine cleaning, I was charged for a periodontal maintenance cleaning. The dental office said that once I receive a periodontal cleaning I will always be charged for periodontal maintenance every time I visit the office even if I do not need it, request it, or decline it. My dental insurance carrier stated the dental office has the authority to stop periodontal maintenance cleaning and charge for routine cleaning. After complaining to the dental office, the office declined to refund the charge for the periodontal maintenance cleaning.

    To whom it may concern, I am 63 and retired and my only income is social security. I have been to many dental offices with S.O.P. Until I went to 17th street modern dentistry a few days prior to 04/05/24 when the Dental work and I gave my insurance information had an exam and x -*********************** the billing manager gave me the results suggested, I procced with dental work that same day, I told her not until my insurance company confirms coverage on 04/05/24 She told me my insurance was confirmed and I requested for printed copy of the complete bill and what is my (complete exact) out of pocket expense, before I go to the chair she circled the amount $896.00 told her, is that it are you sure if not I will walk out the door. She said yes. Before the procedure of the crown, I for cost choosing a basic ********************** out of a curtesy upgraded to the next higher quality. what a kind gesture *************** the procedure was completed, I paid the amount $896.00 on my debit card / Cash. The next day after the injections wore off the crown felt not meshing went in and as a curtesy *************** adjusted my crown. And 08/16/24 four months later I get a bill for ****** the bill has No clarification why or what it is for I go Directly to their office and speak to Destinee the billing Manager no clarification why I owe more. I clearly paid the out-of-pocket amount you circled; you should be honest with people If you told me I might have pay more than what the printed bill clearly designates (which you did not) I would have walked out the door before going to the chair my I paid my $896.00 my balance is (0) Sincerely ********************************* Thank you.
