      I had 1 appointment with Dr. ****** ****** February 25, 2025. Prior to my appointment I sent in my insurance information and got email confirmation that the information was received. Before my appointment ****************** confirmed that they checked my insurance and that Dr. ****** was covered by my insurance. I went ahead with the virtual appointment. However, a few days later, I received a call indicating an issue with my insurance coverage. After spending over an hour on hold with Mindpath Health (the billing company for Dr. ******** I was advised to contact my insurance company directly. My insurance confirmed that Dr. ****** was indeed covered, so I contacted Mindpath Health again. After another lengthy wait, I was informed that while Dr. ****** was covered, Mindpath Health was not, meaning I would be responsible for paying out of pocket for the ************** my surprise, I later received a bill for $784 for a single appointment. This is despite the United Healthcare website indicating that the cost for an out-of-pocket appointment should be $394. I am very concerned about the discrepancy and would appreciate your assistance in resolving this matter.

      I had an appointment scheduled with ****************** of Mindpath Health (*********************************************) for 3/8/24. I was almost out of meds and needed additional refills. We met via zoom. We began the appointment, he asked how I was doing and I said I was well and in ******* for work. He said his license did not allow him to treat me out of ********** so I said that I understood and asked if he would submit a medication refill for me and he did. A few weeks later I received a bill for $50 for a missed appointment on 3/8. As I 100% had this appointment, I contacted the office and said that the bill was incorrect and needed to be adjusted to my $30 copay, what I actually owed. ****************** is insisting that I missed this appointment has continuously attempted to fraudulently charge me $50 for a missed appointment when I did not in fact miss the appointment. I'm aware that he was unable to complete the appointment due to license constraints but that was never made clear to me and regardless, the appointment happened and I received the refill I needed. This is wildly inappropriate behavior for a psychiatrist and his license needs to be reviewed for any further fraudulent activity. As someone who deals with sever anxiety and depression and has been on medication for well over ten years, this is NOT how you treat a consumer with mental health issues.
