      I paid a “new patient eye exam” fee of $335. My insurance company then paid the same fee.
      I requested a refund for my payment.
      I was promised a refund.
      It has now been over two months. I have still received no refund. I am a retiree living on a fixed income.
      This is much needed money in my life.

      Hi ******,

      ***** ******* came to the office for a medical exam w/RFX with DSM on 5/27/22.   Paid 65 dollars for all the Medicare claims.  Medicare was billed with an error in the diagnosis.  Diagnosis must be listed for all Medicare claims.

      Patient received a statement due to Medicare denial and called to speak to billing.  Billing dept rebilled with diagnosis but patient still paid the $335.00.   Patient called again on 8/9/22 and requested a refund.  This request was not sent to our normal refund person but online through our IT refund request portal on 8/9/22.   Patient called again on 9/21/22 asking for status on refund and said he would file with the BBB if he did not receive refund in one week.  A refund was issued on 9/22/22 by Theresa Richards in our Dallas corporate office for $335.00 back to patients' card on file ending in *1754.
