    I have received an email from Elevated Health via Rume Health for a supposed COVID test from 2021. I did use free COVID testing during the pandemic and may have given my insurance info at the time. I did have ********* coverage from ************** possibly at that time, but I never had BlueShield **********. They are claiming I owe them a payment for the balance after insurance for a COVID test. This feels like a scam. It is 4 years later and I have no idea if this is valid.

    It looks like this is not just a one time ordeal. It seems like this company is taking advantage of people. I have never been to this facility. Live no where close to ****************. Recently received a bill for $186 from Oct 22,2021. I have never been here. Didnt even have my current insurance at that time. Yet, somehow this company is trying to get money from my insurance. Definitely not cool. Hope it was an honest, but by the looks of it, it appears they have a habit of committing fraud.

    Today (11/18/2023), I received an explanation of benefit from Blue Shield of California. They claimed that Elevated Health **** had filed a claim for an August 25th, 2021 office visit, which they received and processed on 10/20/2023. I have never heard of this company before. I also did not have Blue Shield of ********** at the date of the visit. I had just started working for a DC-based employer, who only had ******. A quick visit to the business's Yelp review website (***************************************************************************************) shows this is a common occurrence for them, they have more than a handful of reviews from people complaining about mysterious insurance claims. The business owner always claims in response to those reviews that the owner of the company owns a COVID testing/tele-health business, and he uses Elevated Health ***** address to bill for visits to those locations. But the problem is, I've also never used either of those locations, so I feel like this is a strange insurance scam. I would like to:1. Be contacted by the business, 2. Get an explanation for the bill, and 3. For this to be cleared up ASAP.

    I received a denial of healthcare coverage from Cigna health on Oct 30th 2023. This was for a covid test via CovidClinic and original date of service on December 14 2021 from ******************************* DO. I never received any bill from this provider and no bill was sent to Cigna until October 24 2023. Any billing is well beyond 12 months. I do not want to be held responsible due to denial of this claim.

Elevated Health is no longer affiliated with COVID Clinic at all and they are their own separate entity.     I have not yet received a bill, however my previous insurance stated that the request for payment came from Elevated Health and not Covid Clinic.  If you can assure me that you have no outstanding bills for Maisy or Jameson ****** then I would be able to consider the manner satisfied.
