    Billing us for Covid testing back in 2020-21. This is when Covid testing was free. ******** did the Covid testing for ******************** district so kids could go to school and sports. Company send letters saying we would not be billed. Well guess what?? Im still getting billed. Attached is my bill, letter from the company and news about the testing from the school.

    PMH Laboratory, **** was contracted to do weekly COVID testing for my family through my child's daycare, which should have been free to me as a patient. The last day we were tested was 6/1/22. It is now 2/2/23, and I still receive statements with increasing billing amounts due, despite no longer receiving services and having paid all previous balances. On each new statement, the only explanation for the charges is "Refer to Previous Statement". This seems like fraud and should be stopped! I am facing undue anxiety and stress that I will soon be hearing from a collector and my credit score will be affected.Account # **********, Account # ********** and Account # **********

    I needed to get tested for COVID through workplace/employment exposure, and my employer was contracted with **************** for *** testing. Our non-profit agency is contracted with Alameda County (*******) and the lab is based in ****************, **. I have my own insurance provider and we were told that the *** testing would be done at no cost to employees. I was tested on Dec 11th, 2021, Dec 17th, 2021, Jan 13th, 2022 and Jan 18th, 2022 (results which were inconclusive despite me testing positive via rapid antigen). I have been receiving a bill from a collections agency for the past year, with the company initially claiming that I was paid directly/reimbursed by my insurance carrier (I was not), and my insurance took care of the claims directly with the collections agency. Now I am still receiving bills for the amount not paid out because they are claiming duplicate services, or for services not allowed and my ***s clearly state that patients are not to be billed for amounts not covered or overages related to covid testing. I would like for this laboratory to be investigated to determine if they are trying to scam other patients into paying for services when they have already received payment, and despite the policy that the government was providing these services at no cost per the *** statement "G827 THERE IS NO MEMBER COST-SHARE FOR COVID-19 VACCINE OR DIAGNOSTIC SERVICES RENDERED BY OUT-OF-NETWORK PROVIDERS, INCLUDING COPAY, COINSURANCE AND DEDUCTIBLE. THE SUBSCRIBER RESPONSIBILITY AMOUNT DISPLAYED ON THIS *** IS ALSO WAIVED. IF YOU RECEIVE A BILL, YOU MAY CONTACT ** AT THE ***** NUMBER ON THE BACK OF YOUR ID CARD OR BY MAIL TO ***********************************. THE FEDERAL GOVERNMENT PROHIBITS BALANCE BILLING FOR COVID-19 VACCINE RELATED SERVICES." I will have to be on the phone with my insurance carrier again for the 5th time to resolve these claims from nearly a year ago.

    Weekly COVID testing that was done and told was paid by insurance or free from the government and I have been receiving a bill for ******* for almost 2 years. I have given them my insurance multiple times and called multiple times and was told it would be taken care of, yet I am still receiving bills, the most recent one dated 8/1/22
