I was knocked down so hard that I presented into the ER at John F Kennedy Memorial Hospital with sever pain in my left ankle, sever pain and cuts on my right knee, cuts abrasions in my left hand, right shoulder and right elbow.
Then a registration clerk took my insurance information and she said that she was creating a medical record. A few minutes later she brought a tablet over and said that I need to sign something in order to be seen and pay $620.42. She did not give me any copies of what I was forced to sign to be seen
I was never placed on a exam room, nor was I ever examined by physician or registered nurse. I was not given any type of physical exam or given my test results. I did not receive any physician diagnosis or orders.
Several hours later someone came to the Lobby without any introduction and handed my discharge papers and said that he was going to wrap my knee with a ace bandage. I was never received a physical assessment, I was discharged with a paper that said I had a contusion of the right hand, right wrist sprain, contusion of right shoulder, lumbosacral strain, Close head injury, left ankle pain, sprain of talofibular ligament, of left ankle, sprain of deltoid ligament of left ankle, left ankle sprain.

I have reviewed this complaint from an alleged patient that was seen at this hospital. I cannot release information regarding any patient that was seen at this hospital without the appropriate signed consent by the patient. I do see a link in this correspondence that looks like there is some sort of HIPAA consent, but it does not open up for me to view.
