    My wife had had 4 surgeries at *************************** in ************, **. The most recent procedures were dated 05/15/2024 and 07/23/2024. My wife is handicapped, she uses wheelchair, and she is overweight. On 07/23/2024, an incident happened to her at this facility. Prior to the procedure, a nurse transformed her from wheelchair to the bed. When my wife laid down, bed malfunction occured. The bed collapsed. It was tilted at 45 degrees angle, my wife's head was downward. Unfortunately her surgery, that was performed by **********************************, MD, was not successful. My wife needed another surgery. Before the next surgery, on 08/08/2024 we received a news that ****** SURGERY CENTER had decline to accommodate my wife's next surgery because she is a liability to them. We feel like we, especially her, have been discriminated against her condition and disability. Thank you.
