    last statement date 11/24/2021, patient#XXXXX, fee slip #XXXXXX. I spoke with a ****** who would not provide a last name. This ****** is trying to charge me for appointments I never made. This ****** has also accused me of already picking up my glasses and said that I told them I misplaced them. While they are sending me a bill for these glasses. I have never picked up these glasses. They are claiming they gave them to me free of charge at the time with no payment for them. They do not have my glasses, never gave me the glasses, nor did I call to say I misplaced them. I never had an appointment with this ******. I was a walk in with a eye glass prescription from ****** *********** I have a letter from the doctors office dated 11/24/2021 that any fees associated with a missed appointment were removed. My fee should have been a *** ****** Not **** I see the ****** at kaiser for my prescription and only came there to purchases eye glasses. Which I never received. I do not owe this ****** $45 for missed appointments. I even have a letter from their office stating these fees have been removed. The lie that this ****** person claiming I told them I ********* the glasses was ******* and an ****** to my ********** This ****** has never given me the ******* I have proof. I also have proof that the fees were removed. I have proof the prescription came from ******* where I saw the ****** at. They seem to no longer have my eyeglass prescription, yet have charged my insurance carrier. If this provider no longer has my prescription eye glasses they are to return the charge back to **** who are my insurance provider, as well stop trying to bully me into paying them money when they have provided me with absolutely nothing. I owe them nothing they did nothing for me, and they owe my insurance company for glasses I never received. I came in with a prescription in my hand already. I don't owe this ******. He is a crook. I'm ************* and ******** by there attempt to ****** ***

    Business Response /* (1000, 5, 2022/08/16) */ ******* ******** picked up both of her glasses on 6.27.20 according to our records, which have several double checks in place for just such an event. This was over two years ago. She still had a small balance at the time of dispense but we allowed her to take her glasses. MS ******** then scheduled two appointments in 2021 and was advised of our 45 "No-Show" fee. She failed to show or give us any notice that she would be missing the appointments which is very inconvenient for us. She was sent to collections after a third notice for the balance owed on the glasses and one no-show fee. She has a history of ************* and ********** behavior and sometimes uses ********* when ********** the ****** ****** did not want to give her last name for fear of her safety.
