      Date of Incident: July 22nd Amount Charged: $5,600 Service Description:Service Provided: Emergency medical treatment for a fractured ankle *************************** had expired at the time of treatment Emergency Medi-Cal Application: Requested but incorrectly processed by billing assistant Nature of Dispute:Issue with Billing Process: A billing assistant filled out the emergency Medi-Cal form incorrectly and without my consent or signature, leading to a disqualification for emergency Medi-Cal coverage.Professional Conduct: The billing assistant's unprofessional behavior (exclaiming distress at the injury) and handling of the Medi-Cal form.Hospital's Response: Acknowledgement of unprofessional behavior by the billing assistant but no action taken to rectify the situation or respond to follow-up calls.Additional Information:Financial Status: Unemployed for over a year, without benefits, and relying on credit cards.Subsequent Medical Treatment: Underwent surgery, for which I qualified for emergency Medi-Cal.Resolution Sought:Reduction or Elimination of Bill: *** to the erroneous handling of the emergency Medi-Cal application and financial hardship.Contact Attempts: Advised to contact the manager or billing department for financial assistance, but received no constructive response.
