    I was an employee at slimmthicc med spa and I received a **** form from the business after being laid off in late July 2023 due to the spa not being able to pay me or keep me on; as they were not making that much income. I received a **** when I filled out a W2 when he hired me in early April as the esthetician. I never rented a room from the spa, never brought in my own clients, never agreed to a ****. Never was paid accurately nor on time and Im not 100% sure why or if he can legally make me a **** form? It was something I never agreed on

    Details of purchase: I originally purchased a groupon in the amount of $367.08 and my first appt was scheduled for 9/15/2021 at 1pm at 970 Turquoise st. I was advised that the service would not work with just 2 sessions that I had originally purchased through groupon. I asked how many sessions I would need and how much. They had me pull up my shirt, she measured me and took before photos. They got on the ipad and told me it would be 24 sessions (2-3 days a week) and the total would be $3000. Then stated the business was only open on Monday through Friday. I advised I was only available on Tuesdays then. I asked will the service still work if I only come in once week. They stated she would apply the amount paid from the groupon to the total due and that the results would be as if I was cool sculpting but less discomfort and also stated she previously worked for a company that did cool sculpting and she seen better results with your company and also that the results would still be the same coming in once a week. I advised that I would like to cancel the session then because I did not have time or money and that I would go through groupon for the refund since I was not seen. They stated that they had already redeemed the groupon and I would not be able to receive a refund and that they had onsite financing. We did 45 minute session and I couldnt take anymore. The tech scheduled me for 9 more, through the end of the year. I used a total of 8 sessions and it was doing absolutely nothing for me There is before and after photos on their ipad. I made a formal complaint and spoke with ************************* the owner and let him know that I'd like a refund of the unused sessions. ***************** is now telling me that he has a signed contract from me that states no refunds for unused sessions. I asked ***************** to please send me this "signed" contract, 3 times and I have received no response at all. Hes ignored all contact.
