    I spoke w/you yesterday morning about a huge/ridiculous bill I got from an ENT doctor L***** on Casa street,SLO,,,from one apointment I had with him over 10 months ago,,,,I have had some email issues(especially trying to send them),,,so I brought up my old contact with you,,,,,,here is what happened,,,in early October,2021,,,,I had an apt,w/Dr L***** for an obvious sinus infection i had for over 1 month,,,,I called my insurance.whom I no longer use/pay(merick/Salvasen health),,,,to make sure doc was in Network,,,,,Doctor not only charged me $392 for a "new patient/office charge",,,but $410 for a nasal Endoscopy,,,,If I had been pre-informed of the Endoscopy costing me over $200 ,,,I would have got up & left...so a charge of over $400,,,without mentioning anything to the patient,,,is excessive & a RACKET..I saw the doctor in 2016 or 2015 for a sleep study,,,he wanted to do Nose & Throat surgery on me,then,,,I declined,,,,,after the Endoscopy,,,he wanted to do a CT scan on me,,,I declined(glad I did...$$$$)......If you check Yelp reviews on this doctor,,,you will see there are many 1 star reviews,,,complaining about similar issues that I dealt with on this "new patient" apt,,,> rude staph,,,a quirky,aloof doctor,,,,see pages 4 & 5 with reviews from patients Dennis, Ester, & Kirk,,,& others,,,,complaing of him being weird & a "charlatan"/overcharging etc,,,the doctor spent 5-7 minutes with me & wrote a scrpt for antibiotics(which caused me digestive issues not only during consuming them,,,but a few months later),,,,To receive such an excessive bill,,,over 10 months later is DUMBFOUNDING,,,and totally unfair,,,this doctor needs to be thoroughly investigated by the CA medical Board,,,,Im contacting you,,,in hopes that the doctor will drastically reduce this bill,,,,,especially since I was a "patient" about 5 yrs earlier,,,,BUT mostly because they /he never informed me of their excessive charges for such things as an Endoscopy,,,,I assumed my $50 copay for a specialist was sufficent,,,,& judging by all the negative Yelp reviews on him,,,I feel he is ripping people of on a continual basis,,,with his shady practice,,,,,thanks for your help on this frustrating matter,,,,to boot,,,I NEVER received an EOB(Eplanation of Benefits) from my insurance company, for that 10/2/21 visit,,that I was told was "in Network"

    I have never had any communication from this patient; unfortunately his insurance did not respond and a bill was sent to the patient so he could try to get some clarity from them since we were unable to get any information.  I would be happy to discuss this with the patient directly and will give him a call Thursday, August 25.

    I am rejecting this response because:

    I tried to call salvasen health on Friday August 26th,,,,I left 2 voicemails,,,requesting an EOB be processed for my one apt w/Dr Levin on Oct 1st or 2nd of 2021...I never got a response,,,,MY QUESTION is,,,>>>Whose responsibility is it to contact my previous insurance,,& get them to generate/process an EOB for that date of service by the doctor??,,,the company that was insuring me at the time of the visit ...sold out to a different insurer on November 1st,2021(possibly Blackhawk claims?)......to: Dr L***** staff,,,,I have no patient portal & have no desire to set one up,,,so if you need to convey info to me,,,call my landline @ ###-###-####,,,,,this is a very discouraging situation,,,as I paid my monthly premium for health insurance on time every month,,,& made sure Dr Levin was in network,,,prior to seeing him,,,briefly,,,for all of 5-7 minutes,,,& I KNEW I HAD A SINUS Infection,,,did not need a $410 rod shoved up my nose to tell me what I already knew,

    It is ultimately up to the patient as the office billed appropriately and did our due diligence and billed who was active at the time of service.  If you would like to call the office and pay in full I can reduce the amount to the contract rates as a courtesy.
