    I am writing to formally report that I am a victim of identity theft. Unauthorized accounts have been fraudulently opened in my name, which I did not authorize. Below are the details of the account in question:

    I respectfully request the immediate closure of this account and the removal of any associated negative information from my credit report. I am also requesting written confirmation of the account closure and any supporting documentation related to this incident of fraud.

    To assist with your investigation, I have attached a copy of my Identity Theft Complaint.

    This letter is to inform you that I recently received a copy of my credit report that your company publishes and after reviewing it I found a number of items on the report that are inaccurate. The accounts in question are listed below. Please send me copies of the documents that you have in your files as of this date that you used to verify the accuracy of the accounts listed below.

    Under the Fair Credit Reporting Act, 15 U.S.C. § 1681g I have the right to demand that you disclose to me all of the documents that you have recorded and retained in your file at the time of this request concerning the accounts that you are reporting in my credit report. Please don’t respond to my request by saying that these accounts have been verified. Send me copies of the documents that you have in your files that were used to verify them. If you do not have any documentation in your files to verify the accuracy of these disputed accounts then please delete them immediately as required under Section 611(a)(5)(A)(i). By publishing these inaccurate and unverified items on my credit report and distributing them to 3rd parties you are damaging my reputation and credit worthiness.

    A payment of 699.46 was taken from my checking account by this company. It was not authorized by me. I do have an account with this company. they were paid for November in October. On Monday November 18. my checking account was over drawn by $100.00 because of this transaction. I notified the company of this error. I spoke with a representative on Monday morning 11/18. He stated the money would be refunded, and to look out for an e-mail for me to put how I wanted the money. On Tuesday morning 11/19 look for e-mail didn't see one. AT 2pm Tuesday I called GM. A female representative told me my money would be refunded on the 28th. I explained to her I needed before that day for that day. Its Thanksgiving. That wasn't her concern. I spoke with the bank and the money was released to then. Had I known I would have stopped payment on Monday. I NEED MY MONEY NOW! I should not have to wait 2weeks for my funds. They should not have took my money without my permission. I made a in house complaint with them, But the representative told me it would be 10 days before I would hear from them.

    Thank you so much, but it should not ever, have to come to this. I explained...It don't even matter.  I don't even want to say anything else about this company.
