    Argus Home Health was assigned by Developmental Pathways (Dos) to conduct an occupational therapy assessment for home modification. ***************************** (Dr. ** is owner of Accessible Home Modifications PLLC. She came for evaluation on 4/24/24. *ommunication began 2/23/24 client was given pre-interview questionnaire and did not inform client of interview process. Upon arrival client was in care. Dr. * only communicated face-to-face with client's care provider. *are provider told Dr. * client was in bowel regimen. Dr. * insisted she see client. *lient was in bathroom and client told Dr. * she was unavailable but could text with her questions to provide answers. Dr. * took a picture of door needing modification and asked client diagnosis as well as hand function. Despite advisement to ***** client privacy, Dr. * insists she see client again to conduct assessment. *lient asked Dr. * to return at a different time. Dr. * refused. *lient asked **************** to call with further questions and said she would not leave unless documents were signed. A one page document was presented without notes. *lient asked to have documents emailed to electrically sign. Dr. * said that was not possible. *lient signed one page document and was asked to sign ******** document. *lient refused. Dr. * submitted completed forms of assessment to DPs. DPs was informed of privacy and having client sign incomplete documents. Dr. * left client's residence and stated in email to client that she was "noncompliant". Dr. * refused to work with client in rescheduling the assessment. Dr. * is billing clients insurance without client's full understanding due to Dr. *'s neglected to inform client of her participation needs prior and during evaluation. *lient can not be noncompliant. Dr. * is sending HIPPA documentation without encryption/protection of client's personal information. Dr. * is being reported for billing insurance without completed documents. She demanded client signatures on incomplete documents.

    Hi, 

    It is unfortunately *********** did not have a positive experience with AHMPLLC. We alway strive for excellence and a great in-person encounter with our company. *********** unfortunately scheduled her home modification at a time that was inconvenient to her, and seemed to not understand the process of the home modification evaluation. Upon meeting, *********** only wanted to communicate through text message and relay messages through her caregiver. Patient did not want to talk on the phone, be seen, or properly engage in the evaluation. It was communicated to the patient that to be able to comprehensively complete the evaluation, an on site visit was a must. The patient rudely allow the the ** to see her for a 5 second glimpse, and proceeded to communicated via text. This evaluation was approximately 40 mins into the session, when the patient eventually, and rudely asked to reschedule. Unfortunately time was already being calculated and enough information was received to bill for a completed evaluation. The evaluation was completed to ******** satisfaction, with as much information that was received and required. The patient was attempting to get out of being billed for services and time already completed because she was upset. Unfortunately time with a patient still needs to be recorded and billed. Of our knowledge, the patient signed the paperwork. The patient was emailed copies of the evaluation and was told she has the right to no go forward with the modification.

    I am rejecting this response because: OT Dr. ****** assessment was incomplete, ******** was improperly billed, OT was not present for the time specified in response compliant or on state based paperwork. Client was Evaluation did not suit client's needs and Dr. ****** is using slander and unprofessionalism in/outside home of client. Client was victim of billing scam. Dr. ******** claims are false and client has witness.

    State based paperwork is falsely reported as one hour of an assessment when client was not seen or properly seen or evaluated, Charges were not specified by ***************************** (OT contractor with Argus Home Health). These blink camera shots confirm she has lied to the client, ******** and BBB.

    Again, we are sorry to hear *********** did not have a sucessful experience with AHMPLLC. All claims for false billing, scams, and incomplete evaluations are not true, for this is a ** LICENSED professional contracted business, all given services and the allotted times are closely monitored. Unfortunately this is the experience of ONE individual person, compared to the plethora of patients and clients this company has worked with, that has had nothing shy of excellent expericences, services and outcomes. *********** was givien the option to not move forward with the modification, and chose to do so. All buisness between this patient and AHMPLLC has been completed, and no other contact with this patient is required moving forward.

    I am rejecting this response because: there is evidence of false time allotted and billed to my insurance. **************** is blatantly ignoring her business ethics. She has falsely reported her time and services and did not complete any assessment of client. ****** saw her through a small gap in the door while she was given a bowel regime.
