- Cancellation – If you wish to cancel your plan, please call the customer service number shown below. If you are paying monthly, your plan will be cancelled at the end of the month in which the cancellation request is received. If you selected to pay annually, your plan will be cancelled at the end of the month in which the cancellation request is received and we will refund any unused portion of the annual payment. You are responsible for making payment for the months during which benefits are in effect.
