      I'm writing in reference to Synergy Medical, LLC located in ********* *******. The said medical provider is a contracted service provider with **************** As a contracted provider they agree to accept the insurance company's agreed upon reimbursement rate for the servicesprovided. In addition, as a contracted service provider they are not allowed to balance bill the patient. They can only collect from the patient the assigned patient responsibility according to the Explanationof Benefits (EOB).Synergy Medical, LLC, is not abiding by the contractual agreement they entered into when they agreed to be a contracted service provider with Cigna. They have been provided with the EOB's from Cigna as well as myself. They have been contacted multiple times by myself detailing the patient responsibility, what was paid upfront prior to service being provided as well as the reimbursement/refund owed me. I'm always skeptical about prepaying medical service providers for fear of not being reimbursed if I overpay. However, Synergy reassured me that if I overpaid after the insurance company paid, I would be refunded. I even signed a document ["Refund Calculation Agreement" ] for which that statement was included, yet I have been struggling for two years to get the full refund owed me.
