    I am still waiting for the refund that is due to the insurance discrepancies in co-pays that I was being overcharged. Attached you will find a spreadsheet of the insurance charges and payments. Please note that because you did not send the insurance company the necessary documents they requested for the Occupational Therapy Evaluation on May 4, 2022 that you cannot bill me for those services. The original payment of $240.00 made on June 21, ************************ out with a $120 credit to the account. The first visit on September 8, 2022 did require another $40.00 co-pay which would have balanced out the account. All of the appointments during the remainder of 2022, you will see were paid by the insurance company in full rather than requiring the $10 co-pay. That would leave me with a credit on the account of $1,140.00 going into 2023. You were originally billed as out of network which is where the $65 p/visit came from, but note that this was corrected to a $15 co-pay. After 11 visits in total for 2023, $165 in co-pays were due. This leaves me with a $975 credit that I would like refunded by check. On December 9th, 2024, I finally received the following email: After reviewing your claims, we have identified that while services were provided, several claims were denied due to insurance and administrative errors. These denied claims are not your responsibility. Based on this, you are owed $1015.00. Accepting this refund confirms your agreement that the issue is fully resolved and will not be reopened with our office or your health plan. Please confirm your acceptance of this resolution. If accepted, please provide the name the check should be made out to and the proper mailing address. Thank you for your understanding as we work to finalize this matter. I noted acceptance and confirmed my mailing address that same day. On December 31, 2024, I sent a reply email asking when I could expect the check and I have had not response to that email.
