      On April 18, 2023 I was seen at Orthopaedic Medical Group of Tampa Bay. Once I arrived, I was told they wanted to have x-rays done. (I was not informed of this prior to my arrival.) Before allowing the x-ray to be done, I told the staff I had already recently had x-rays and an MRI done. The office ************* me I would not be charged any additional fee for the x-ray, and all I owed was the $10 I paid during the visit, and my insurance company would cover the cost of the x-ray in full along with the other fees. On June 1st I received a bill from them stating I owe $24.47. It was for the x-ray. I immediately called the phone number listed for their ******************* office and reached another location instead. I was told I would need to speak with the billing office. I spoke with **** who informed me the ******************* office would have to communicate with her to get the charge removed, and that there wasn't any note in my account regarding what I was told regarding the x-ray fee during my visit. She said I would have to speak with the clinic manager who is usually unavailable. I was then transferred to the clinic manager's voicemail. I left a message. I followed up with a written request the same day. As of today June 8th I still haven't received a reply, so I tried contacting both the ******************* clinic manager and the billing manager again. I spoke with ****** in the billing department who said she would need to transfer me to their receptionist line and I would need to ask for the clinic manager. I was transferred to someone named **** who said he couldn't transfer to the clinic manager. I called back the billing department again. This time I was told a note was put in my account stating I was told by their ******************* staff that no guarantees would be made regarding payment, which is the complete opposite of what I was actually told. Then I was told I would need to speak with the billing manager named *******. I was transferred to another voicemail.

      **************** had an appointment on 4/27/2023 with our neurosurgeon. The x-rays that were provided at the time of the visit were not sufficient or the medical specialist who requires specific views to assist with his diagnosis and treatment plan. With an understanding that every insurance is different and our practice accepting over 200 plus different insurances, Orthopaedic Medical Group of Tampa Bay makes no guarantees regarding what a patient may owe once a medical insurance company has been billed. The additional $14.47 over her $10 copay was from the patient's coinsurance policy that was billed by ********. On 6/19/2023, $14.47 was removed from the patient's balance as a one-time courtesy adjustment.

      I am rejecting this response because:

      Along with the billing adjustment, treatment notes need to be added to my April 27th Visit Summary, via the patient portal. The doctor gave me a lot of information verbally during the visit but nothing was put in the after-visit summary notes. (Treatment notes were included in the April 18 visit summary but not the April 27th summary.)

      Unclear of what's being requested as the clinical notes from 4/27 state that a discussion was had regarding providing names for outside providers such as chiropractors and acupuncturists to seek further conservative treatment as surgical intervention was not requested. There was also discussion of heel lifts. This can be found in the Assessment portion of the note (not provided here for privacy reasons). Any treatment plan would be provided by these providers. At the 4/18/2023 visit,it's noted that there was no interest in injections, surgery, or physical therapy which is why other options were provided in the 4/27 assessment.

      Only the diagnosis name and code are listed in the 4/27 visit summary. I've attached screenshots here to show how the notes were done for the first visit but not for the 2nd visit.

      Screenshots provided are from a limited view of the patient's record. Please see attached redacted screen shot from the actual medical record which we would be happy to send. The neurosurgeon did not provide a treatment plan for this visit as the instructions were to follow up PRN ("pro re nata" or as needed). Any future treatment plan should be provided from future providers.

      The issue I was referring to in my prior message with the screenshots was not regarding a treatment plan; it was regarding ************** not putting in the notes that were discussed during the visit like Dr. ****** had done. The two screenshots I sent show what I am referring to, how Dr. ****** added specific notes and ************** did not.

      Also I'm still waiting for a reply to the message I sent you in the patient portal regarding my x-ray images done at your clinic. The x-ray images you sent were the ones that were done by Tower Radiology back in December, not the ones done at your clinic.
