    I had been seeing an in-network provider for over a year, with a copay for appointments. I was informed a few months before the date that the provider would no longer be in network. I had an appointment with that provider scheduled after that date, and when the billing person, *******, called, as usual before the appointment, he said multiple times, when I asked about my insurance covers for the visit or if it would be private pay, that this would be the last time it would be covered by my insurance and the copay was the usual $30. I started to receive bills for this visit stating my insurance didnt cover it. I have emailed and called and written the company multiple times as their bill states to do if you have a problem with the billing, but no response. The most recent billing statement I have shows they took two payments out of my account they were not authorized to do on 2/4 for $12.74 and for $17.26. The amount they state is owed is $91.50. I was billed for the evaluation and management of an established patient, which I had, and for psychotherapy with established patient for ***** minutes, which I did not have. The med management was billed at $92.31, which would be what the $30 copay was for, that I authorized payment of, but they took $60 out of my account- with the $30 coming out on 1/31, and the 2 unauthorized payments as noted above. The psychotherapy which I did not have was billed at $59.19. I have asked multiple times for them to pull the call if it was recorded to hear the times I asked to confirm the coverage and what their employee, *******, responded. If you need me to get the emails I have sent via their portal or through my email, I can send those to you, attached are the written statements sent to an address in ***** if you are disputing the bill.
