    I had appointment on 7/13. Appointment lasted beyond 5pm and I was rushed by staff when completing follow up care. It was agreed that I would get a PET scan near my home. I received approval for the scan on 7/19. My wife called on that day and every day there after to have the approval and script sent faxed to PET/CT of FL. Beverly Hills FL fax *************. I also send them emails requesting the same. They said they would fax the documents however they failed to complete this simple task. Additionally on 7/13 I gave DR T***** notes and scans fro my Neurologist in Ocala. It was my only copy. Being rushed out of the office I failed to ask for a copy that I need for other health providers. My wife called the office on 7/14 and was told they would mail a copy to us. I have yet to receive the documents or acknowledgment that they have been sent. Please help me in getting these simple tasks completed.
