      My daughter, who was 17 at the time of services rendered, had received counseling from Chrysalis health through Hillsborough County schools. This was a free service that they provide to a limited number of students at the school. We never signed up for any additional paid services. For several months now we have been receiving bills for $840. I have called the number provided on the bill and left messages and I have never received a call back from them. I explained the whole situation in the voice messages sent to the billing department. I also asked the counselor who rendered the services about it and she told me that I have to contact the company themselves and that she has nothing to do with billing. My problem is that the company ignores all of my attempts to get into contact with them and they are sending me bills for a free service.

      in 2020 and early 2021 we received services from Chrysalis health. By June 2021 between insurance payments and personal payments somehow Chrysalis had made quite a mess of our accounts and I spend a lot of time on the phone with them to sort it out. I paid the remaining balance of 85.00 in June 2021 before moving on.
      Now in Feb, 2023, they reach back out saying they are going to send us to collections if we do not pay an additional 781.00. This is not my issue, I've already settled with them, but now there is a whole new group of employees on the email and I no long have access to my old email with all the correspondence. The staff only points me to filing some paper complaint, which I will do. I also understand in Floriday they have 5 years to come back and bill, but this is not a true amount due.

      My daughter had an appointment for psychiatric evaluation code 90791 PRVT-DX EVAL. Right after the evaluation finished, I was told hat I would receive a call no later than 2 weeks for an appointment, This was back in Nov 27 and this never happened. She had a diagnostic over the phone that day that needed their attention and I followed up multiple times by email and phone after that for months and never got the appointment to be treated. However, I have received over 20 bills that I refused to pay because what is the idea of obtaining a evaluation for which I never received a copy although I have requested in multiple occasions to both Jennifer Perez and Lya Castillo and with no success. I requested the billing department to waive the charges as this was never treated due to them being irresponsible and not following up with me fo ran appointment and not even giving me a copy of the report. Why would I pay for something that I don't even have in my hands and they have failed to provide? They don't respond to emails or Voicemails, I need them to kindly send me her evaluation and to remove the charges of $118.19 under Account number nwbivaq3mp.
